Tranexamic acid (topical)

Also known as: TXA, trans-4-aminomethylcyclohexanecarboxylic acid

Tranexamic acid is a repurposed anti-bleeding drug that turned out to quiet the signaling between blood vessels, UV-stressed skin cells, and pigment cells that drives melasma. Oral TXA has the strongest data; topical 2 to 5% versions now appear in serums and have randomized trial support showing meaningful pigment reduction, in several comparisons performing near hydroquinone with less irritation. It is one of the more promising newer pigment ingredients, though topical trials remain small and mostly short.

How it works

TXA blocks plasminogen activation in skin. Less plasmin means fewer prostaglandins and less vascular endothelial growth factor, which are the messengers that tell melanocytes to overproduce pigment after UV exposure. It attacks the upstream signaling of melasma, including its blood-vessel component, rather than directly bleaching pigment.

These are the results measured in studies, not the results shown in ads. Where the research is weak, the grade says so.

For melasma

Moderate evidence

A 2024 meta-analysis of randomized controlled trials found TXA significantly reduced melasma severity (MASI) across routes, with oral dosing strongest, followed by injected and topical use. Reviews of comparative trials report topical TXA achieving pigment reduction comparable to hydroquinone with fewer irritant reactions. Most topical trials run 8 to 12 weeks at 2 to 5%; the number of high-quality topical-only RCTs is still modest, which keeps this below a strong grade.

Randomized trials support this use, but they are few, small, or short. The effect is probably real. The size of it is less certain.

Cosmetic brands increasingly pitch topical TXA for all dark spots and post-acne marks, but the published evidence outside melasma is thin. A 2026 literature review of TXA in melasma and post-inflammatory hyperpigmentation found supportive results but leaned heavily on melasma trials, with PIH data limited to small clinical and observational studies. Reasonable to try given its gentleness; not yet proven for general dark spots.

Only small, short, or lower-quality studies exist. Treat marketing claims about this use with caution.

Risks and what can go wrong

Topical TXA is notably gentle: mild transient redness or scaling in a minority of users, and systemic absorption from skin application is negligible, so the clotting-related cautions that apply to oral TXA (history of blood clots, some hormonal contraception combinations) are mainly theoretical for creams. Safety in pregnancy has not been formally studied topically, so most clinicians defer it. Oral TXA for melasma is prescription-only and off-label.

The practical details

OTC in serums and creams at 2 to 5%, often combined with niacinamide or kojic acid. Applied once or twice daily; visible change typically takes 8 to 12 weeks. Works best alongside rigorous sunscreen use and can be layered with most other actives. A practical maintenance option after a hydroquinone course.
